The services provided are individualized, tailored to the specific needs of each person. Clients must meet the following criteria to qualify for WiderTouch Private Duty Nursing services: - Have a documented medical need for continuous skilled nursing care. - Be under the care of a licensed physician. - Reside in a safe and appropriate home environment for care delivery. - Have authorization from a payer source/person. Accepting only out of pocket Payments at this time. - WiderTouch Private Duty Nursing (WTPDN): Skilled nursing care provided on an hourly basis to medically fragile individuals in their homes. WiderTouch aims to serve as a liaison to facilitates the continuity of care and bridge life's transition with limited interruptions; thereby allowing for rest and healing to take place. - WiderTouch Skilled Nursing Care: Concierge Services that require the clinical judgment and expertise of a licensed nurse, such as medication administration/ reconciliation, tracheostomy / Gastrostomy Tube care, Diabetic/ Hypertension management and Education, Insulin injections, Vaccines injections, wound care, Wellness Checks,
